This event will look at how we can connect housing services, particularly housing-related support, to health and care.

Making better connections between health, housing and care is a significant challenge facing our sector. This event will look at how we can connect housing services, particularly housing-related support, to health and care.

In order to improve people's access to health there is need for even further collaboration between health, housing and social care. Cymorth Cymru is in a unique position, able to link different policy areas through the shared support needs of the people our members work with.

We know that services must make every contact count if they are to be effective, using each possible opportunity to identify people's health and housing needs and help them to access appropriate support.

With this in mind, we begin the new year with a conference agenda that draws together the strands of policy and practice around health, housing and care and asks: how can we connect this all together?

A full day of talks, workshops and debates on how to build connections across policy areas for practitioners and the people we support. Spaces are limited and Cymorth's recent events have reached capacity - we recommend booking your place quickly.

There will be workshops, debates and plenary sessions on the following topics:

• Encouraging integration between health, social services and the housing sector
• Working differently to improve the health of people experiencing homelessness
• Developing a solution to the challenges of substance misuse in supported housing
• Addressing loneliness and isolation
• Positive Behaviour Support, ACEs and trauma-informed approaches
• Providing support for people struggling with hoarding disorder
• Excessive alcohol use in people over 50
• The integrated care fund and avoiding institution
• Public health challenges around marginalised groups

Expertise and debate will come from from experienced representatives of Public Health Wales, various Health Boards, local authorities, housing associations, third-sector organisations, government programmes such as Well North Wales, national partnerships and campaigns including ACE Support Hub Wales and Drink Wise, Age Well.
