Gwalia Care & Support

Person A lived in foster care before moving into supported housing. Person A has been diagnosed with physical and mental health issues. Since receiving support, Person A registered with a GP and a dentist. Gwalia accompanied Person A to regular appointments with her GP and other medical appointments.

Person A accessed counselling advice and support. Self-harm advice was given. A referral to the NERS (national exercise referral scheme) via her doctor. Person A has now moved into a Gwalia flat in an area which she is familiar with and is receiving floating support to help with resett...

Gofal Caerphilly 2

Person A had rent arrears for which CCBC were looking for possession of her tenancy. She has bipolar disorder, anxiety and experiences panic attacks.

Person A has four children living with her from ages 1 to 16 years old and when referred to Gofal was facing the possibility of further action being taken on her tenancy if she hadn't started paying her rent and arrears which eventually would have been eviction.

Gofal Caerphilly 1

Person A was referred to the Gofal Caerphilly crisis team via Pathways on receipt of a NSP. Person A had previously lived in the property with her parents and grandmother who have all passed away, leaving Person A in the property alone. Person A has severe anxiety and depression and is also prone to self-harm.

Since being referred, Person A now manages their money and accommodation whilst receiving support for their mental health, with plans to attend a Health and Social Care training course and continue to engage with Gofal tenancy support and pathways.

X is a female between the ages of 40yrs-50yrs. She was referred to Mind the end of November last year. X is fairly isolated with limited social networks. X suffers with mental health issues and is wheelchair bound due to her mobility issues. X is also alcohol dependent.
